Job summary

Nourishing Hope in Chicago is seeking a Social Services Coordinator to support SOP execution, ensure high-quality programming, and maintain accurate case records in Azzly and Salesforce. You will coordinate access to healthcare, crisis intervention, housing, employment, and benefits, supervise interns, and serve as the primary contact at food distribution sites with neighbors and volunteers.

Qualifications

• 1+ year of work experience in social services (clinical and case management setting preferred).
• Bilingual (Spanish) required.
• Strong technology skills: Microsoft Office & Google Suite.
• Familiarity with Salesforce CRM data quality.

Responsibilities

• Support execution of SOPs to ensure quality programming and safety.
• Supervise interns as appropriate.
• Point of contact for neighbors, volunteers, staff at food distribution sites.
• Coordinate access to healthcare, crisis intervention, financial, housing, employment supports.
• Maintain accurate case records in Azzly and Salesforce; administer IM CANS assessments.
• Participate in Mental Wellness & Social Services team and align with organizational priorities.
• Attend external meetings and trainings as required.
