Job summary

Nourishing Hope in Chicago seeks a bilingual Case Manager to connect neighbors with critical supports and ensure dignified, respectful service in line with our core values. You will coordinate access to healthcare, housing, benefits and transportation, maintain case records in Azzly and Salesforce, and collaborate across the Mental Wellness & Social Services team to meet organizational goals.

Qualifications

• Bachelor’s degree in social work or related field.
• 1+ year of work experience in social services environments.
• Bilingual Spanish required.
• Strong tech skills: Microsoft Office, Google Suite, Salesforce.
• Commitment to Nourishing Hope’s mission and food security for all.
• Familiarity with barriers faced by neighbors experiencing food insecurity.
• Excellent interpersonal and communication skills.
• Ability to work independently and as part of a team.

Responsibilities

• Serve as liaison with neighbors, volunteers, staff and stakeholders.
• Coordinate access to healthcare, crisis intervention, housing, transportation, and benefits.
• Maintain accurate case records in Azzly (EHR) and Salesforce; administer IM CANS assessments.
• Ensure SOP-aligned social services delivery and data quality.
• Collaborate with Mental Wellness & Social Services team and participate in external trainings.
