Job summary

Nourishing Hope in Chicago seeks a bilingual Case Manager to connect neighbors with essential supports, maintain accurate case records in Azzly and Salesforce, and ensure quality service aligned with our core values.

The role is Monday–Friday, 9:00 AM–5:00 PM, mainly at Sheridan Market with one day weekly at El Mercadito; onsite with occasional remote work as needed. Fluency in Spanish required, with compassionate communication.

Qualifications

• Bachelor’s degree in social work or related field.
• 1+ year of work experience in social services (clinical and case management preferred).
• Bilingual Spanish required.
• Strong technology skills including Microsoft Office Suite and Google Suite.
• Ability to use Salesforce CRM data quality.
• Commitment to Nourishing Hope’s mission and understanding of food security.
• Familiarity with barriers faced by neighbors experiencing food insecurity.
• Strong interpersonal and communication skills and professionalism.
• Ability to work independently and in a team, with flexible coverage.
• Ability to prioritize workload in a fast-paced environment.

Responsibilities

• Support the execution of Social Services SOPs to ensure high-quality programming and data quality.
• Act as the point of contact for neighbors, volunteers, staff, and stakeholders at distribution sites.
• Ensure high-quality Social Services programming via outreach, appointments, and drop-in hours.
• Maintain accurate case records in Azzly and Salesforce and administer IM CANS assessments as appropriate.
• Participate on the Mental Wellness & Social Services team and support cross-department alignment.
