Job summary

Qualifications

• One year related experience and/or training.
• Ability to work independently with great attention to detail.

Responsibilities

• Assess clients' needs and manage ongoing case work.
• Conduct intakes and provide information and referrals.
• Advocate for clients and arrange provision of services.
• Maintain complete and accurate case notes.
• Facilitate and assist at social and group activities.
