Job summary

You will conduct home studies, provide trauma-informed post-release services, and connect families to community resources.

Travel up to 65% is required; bilingual English/Spanish is essential. The role emphasizes confidential case management, collaboration with supervisors, and commitment to faith-based service.

Qualifications

• Bachelor’s degree in Human Services, Social Work or related field and required case management experience.
• Bi-lingual English and Spanish; diverse client service ability.
• Strong verbal and written communication; confidentiality and independence.

Responsibilities

• Provide culturally and linguistically appropriate Post Release Services and case management.
• Maintain a caseload per contract terms and agency expectations.
• Conduct home visits and home study assessments as needed.
• Refer clients to resources, advocate for services, and document in the database.
• Collaborate with supervisor and community partners to support families.
