Job summary

Qualifications

• Two years of direct services in Mental Health or community services; or Associates degree with one year of experience; or Bachelor’s degree in Social Services.

Responsibilities

• Serve as primary support and case manager for individuals with mental illness and co-occurring disorders.
• Assist clients to live as independently as possible in their chosen community.
• Provide case management under direct supervision.
• Coordinate services and supports in the community.
