Job summary

Wesley seeks a Case Manager in Houston to conduct client intake, assess eligibility for emergency financial assistance, and develop self-sufficiency and housing stabilization plans. You will connect clients to resources, maintain records, and advocate for access to services within the Community Services Department.

Requirements include a Bachelor’s in Human Services and at least two years of relevant experience, with bilingual English/Spanish skills and strong computer literacy.

Qualifications

• Bachelor’s Degree in Human Services and at least two years of client-facing experience.
• Bi-lingual in English and Spanish with clear written and spoken communication.
• Strong computer skills including typing and Microsoft Office.
• Ability to work cooperatively and support team efforts; attention to detail.

Responsibilities

• Recruit, screen, enroll, and work with clients who qualify for Wesley programs.
• Complete initial program assessment to determine eligibility for services.
• Provide one-on-one case management services and advocate for access to services.
• Maintain confidential client files and accurate HMIS/Apricot documentation.
• Develop individualized Housing Stabilization Plans and coordinate HQS inspections.
• Track grant goals and assist with reporting; ensure timely disbursements to providers.
• Engage in community awareness and represent Wesley at events.
