Job summary

Casa Esperanza, Inc. is a bilingual behavioral health provider in Boston offering comprehensive case management in the LyN Residential Treatment Program.

You will coordinate medical, behavioral health, social, and legal resources to support patients and their children in achieving recovery goals. You will collaborate with a multidisciplinary team, document progress, and provide crisis intervention while ensuring compliance with regulations.

Qualifications

• Two years of experience in social services with a substance abuse population.
• Certified Alcohol and Drug Counselor/ability to obtain within 1 year of hire.
• Bilingual Spanish/English with cultural insight.
• CPR Certification required.

Responsibilities

• Provide ongoing case management and care coordination for patients and their children.
• Develop and monitor patient-centered service plans with the treatment team.
• Coordinate medical, behavioral health, and social services appointments and transportation.
• Collaborate with internal and external providers to ensure timely access to services.
• Manage DCF-related responsibilities, including communication and reunification planning.
• Refer to community resources (housing, employment, childcare, etc.).
• Assist patients in meeting legal obligations (court, probation, etc.).
• Facilitate psychoeducational and recovery-focused groups.
• Participate in team meetings and treatment planning.
• Maintain timely documentation in the electronic health record.
• Provide crisis intervention and patient advocacy.
