Job summary

Join New Hope Clinic in Kennewick, WA as a Case Manager with Yakima Valley Farm Workers Clinic. Build program-specific caseloads by coordinating referrals and conducting intake screenings to identify client needs and eligible services.

You will perform in-depth interviews, refer clients to appropriate agencies, and assist with self-referral and service plan development. Regular in-home visits, collaboration with the team, and regulatory compliance are key aspects of the role.

Qualifications

• Bachelor’s or equivalent in social services or related field (required for CHS Maternity and Infant Case Manager)
• Associate’s degree with 2 years in community social services or related field
• High school/GED with 4 years in community social services or related field
• Bilingual differential compensation when ALTA score ≥10
• Bilingual (English/Spanish) preferred at high level 10
• Valid Driver’s License and automobile liability insurance
• Ability to travel to surrounding YVFWC sites
• Strong verbal, written, and listening communication skills
• Knowledge of local social and health services
• Experience with Microsoft Office programs

Responsibilities

• Build program-specific caseload by seeking referrals to meet program objectives
• Conduct intake screenings to assess clients and determine services
• Conduct in-depth client interviews to determine status, barriers, and needs
• Refer clients to appropriate agencies and services
• Assist clients with self-referral process
• Meet regularly with clients, often in-home, to monitor service plans
• Create and maintain client service plans and documentation
• Maintain relationships with service providers and stay informed about resources
• Participate in interdisciplinary team case reviews
• Perform clinical responsibilities per TJC, HRSA, and YVFWC requirements
• Perform other duties as assigned
