Urban Health Plan in Bronx, NY seeks a Case Manager (Bilingual English/Spanish) to support behavioral health patients. You’ll provide social services, screenings, internal/external referrals, and advocacy to improve access to needed resources.

Entry-level position requires a Bachelor's degree and fluency in Spanish and English. Onsite, Monday–Friday, with comprehensive benefits and a focus on patient-centered care.

Qualifications

• Bachelor's degree in social work, psychology, human services or related field.
• Entry level case management position; no prior experience required beyond degree.
• Bilingual Spanish/English required.
• Team-player and committed to quality patient service.
• Proficiency with Microsoft Office.

Responsabilités

• Provide case management support to patients, including assigned populations.
• Draft letters for public assistance, housing, IRS, schools as needed.
• Assist with completion of prior authorizations.
• Connect patients with community resources based on site needs.
• Advocate for patients to obtain targeted services.
• Collaborate with colleagues to ensure patient support.
