The role focuses on providing letters for public assistance, housing, IRS, and schools, plus linking patients to needed resources. Entry-level position with a Bachelor's degree; strong emphasis on teamwork, Microsoft Office proficiency, and understanding of medical terminology.

Qualifications

• Bachelor's Degree in Social Work, Psychology, Human Services, or related field.
• This is an entry level case management position; no previous experience required beyond Bachelor's Degree.
• Team-player and willing to go the extra mile to support patients in our care.
• Proficiency with Microsoft Office.
• Understanding of medical terminology and social work principles.
• Commitment to excellence and to providing quality service to patients.

Responsibilities

• Provides case management assistance to patients including populations as assigned.
• Provides letters to patients including letters for public assistance, housing IRS, schools.
• Provides community based resources to the population based on site/ department/ program need.
• Advocates on patients behalf to obtain specific resources or services including specific or specialized services.
• Works closely with departmental coordinators and team members to provide patients with support.
• General case work responsibilities, supporting additional duties as assigned.
