Job summary

Care Continuity, Inc. is seeking a Bilingual Care Navigator to help patients coordinate post-discharge follow-up care. You will contact patients in English and Spanish to schedule appointments and, when needed, call physicians’ offices to book visits.

Qualifications

  • Fluency in English and Spanish, both spoken and written.

Job description

Care Continuity is hiring a Bilingual Care Navigator, fluent in English and Spanish, to help patients get the follow-up care they need after leaving the hospital.

You'll contact recently discharged patients and help them schedule their recommended follow-up appointments. For patients who accept our services, you'll call their physicians' offices to book those appointments. Your work helps patients make a smooth, healthy transition from hospital to home.This role suits someone who enjoys working independently on daily assignments, pitches in to support the team, and cares about patients. What You'll Do
  • Make and receive calls with patients and provider offices in English and Spanish
  • Schedule follow-up appointments on patients' behalf, based on their preferences and availability
  • Review patient charts (written in English) to understand follow-up recommendations
  • Verify insurance coverage using online tools and resources
  • Send and receive medical records by fax
  • Write clear, detailed notes in English on patient scheduling preferences and call outcomes
  • Manage your daily workload independently and support teammates when needed
What You'll Need

Nice to Have

  • Experience in healthcare, a medical office, a call center, or customer service
  • Familiarity with medical terminology or health insurance
