Job summary

Wheeler Health & Wellness is seeking a Care Manager in Plainville, CT to welcome new patients, monitor outcomes, and coordinate health screenings and external referrals. The role includes community engagement and ensuring access to Wheeler services within an integrated primary care model.

The position emphasizes patient-centered care, collaboration with the care team, and adherence support while addressing social determinants of health and resource navigation across the community.

Qualifications

• Bachelor’s in Social Work, psychology, or other human services related field plus at least 1 year of related experience including face to face interaction with clients is required.
• Care coordination or case management experience preferred.
• Knowledge of Connecticut community resources and organizations; CT driver’s license required; Spanish speaking preferred.

Responsibilities

• Welcomes and engages all new patients to the health center.
• Attends local community collaborative meetings and arranges services based on care plan.
• Monitors patient adherence to Care Plan and coordinates wraparound services.
• Coordinates care through meetings to facilitate services and resources.
• Documents care coordination efforts and referrals.
• Supports social determinants of health screenings and navigation to care.
