Job summary

Qualifications

• Associate degree or equivalent in human services or related field.
• Minimum two years experience with logistics coordination and outreach/case management.
• Valid driver’s license and availability of an insured vehicle.

Responsibilities

• Screen individuals or families using a standardized tool.
• Provide educational materials and community referrals as needed.
• Market the Healthcare to the Homeless program in the community.
• Attend community collaboration meetings, workshops and events.
• Coordinate administrative functions for smooth patient flow and high service quality.
• Uphold program continuity and develop relationships with community sites.
