Job summary

Wheeler Health in New Britain, CT seeks a Care Coordinator to provide case management, direct services, and care coordination for an assigned caseload of individuals and families. The role may facilitate groups while coordinating with families, DCF, schools, and service providers.

The position requires a BA/BS in a related field, experience with mental health or co-occurring disorders, strong communication, and a valid driver’s license.

Qualifications

• BA/BS degree in Psychology, Social Work, Marriage and Family Therapy, Counseling, Criminal Justice or related field is required.
• Previous experience working with clients with mental health and/or co-occurring substance use disorders is preferred.
• Effective time management, strong verbal and written communication skills are required.
• Valid Driver's License is required; operates vehicle safely in the community.

Responsibilities

• Develops case management plans and coordinates care for assigned clients and families.
• Provides front line crisis support by phone, in-home, or in the community as needed.
• Maintains documentation for client medical records in accordance with timelines and regulatory requirements.
• Coordinates treatment with internal and external providers and supports as appropriate.
• Meets direct service productivity goals and adheres to program guidelines and data collection.
