Eliot Community Human Services, Inc. is hiring a Bilingual Care Manager for the CSA program in Washington. This role focuses on child and family behavioral health, crisis management, and coordinated care planning in homes or family locations.

The ideal candidate will coordinate individualized care plans, assist with system navigation, and provide ongoing family support, leveraging cultural sensitivity and strong communication skills.

Qualifications

• Bachelor’s degree in human services or related field.
• Valid driver’s license and reliable transportation.
• Experience navigating child/family service systems and crisis management.
• Fluency in Spanish, Haitian Creole, or Portuguese.

Responsabilités

• Serve as key contact for the youth and family to the Eliot CSA and its services.
• Initiate care planning activities with all members of the family and/or caregiving system.
• Conduct a comprehensive home-based assessment with CSA Team members, using tools as determined necessary.
• Initiate planning that includes intake meetings and orientation to the wraparound process.
• Develop and implement a youth and family centered ICP in collaboration with the family and collaterals.
• Develop an initial safety/crisis plan.
