Job summary

Person Centered Services Care Coordination Organization, LLC is seeking a Social Work Professional to work within a health home model serving individuals with intellectual and developmental disabilities.

The Care Coordinator, also known as a Care Manager, collaborates with health care and community providers to ensure needs are met and coordinates services including health, behavioral health care, and supports.

Qualifications

• Bachelor's degree with 2 years of relevant experience OR a Licensed Registered Nurse (RN) with 2 years relevant experience or Master's degree with 1 year relevant experience
• Minimum of a Bachelor's degree is required by OPWDD in NYS
• Relevant experience includes direct care or support, coordination, and/or advocacy for anyone (examples: Direct Support Professional, Teacher, Paraprofessional, Counselor, Social Worker, Care Manager)
• Valid driver's license required
• Bilingual in English and Spanish required

Responsibilities

• Develops a comprehensive, person-centered Life Plan with the individual and their circle of support and entire service provider team
• Effectively manage a tiered caseload, tailoring services to individual needs
• Completes enrollment and eligibility documentation
• Meets with individuals in their homes, physician/provider offices, and other public places to conduct assessments and provide services
• Commits to a respectful, just, and supportive environment aligned with diversity, equity, and inclusion
