CSI, a Help at Home Company, is seeking a Care Coordinator in Summerfield, FL fluent in English and Spanish to support clients and caregivers. This role focuses on caregiver scheduling, care coordination, and delivering excellent client experiences.

You will maintain files, communicate schedules, conduct in-home visits, and ensure HIPAA compliance while partnering with branch managers to uphold quality home health services. This is a full-time, on-site position with travel requirements.

Competenze

• High school graduate or equivalent; higher education or certification may be required.
• Fluent in both English and Spanish.
• Basic computer literacy and typing skills.
• Experience in health care, preferably in home health; supervisory experience.
• Medicaid, Waiver, or Home Healthcare experience preferred.
• Other requirements pursuant to state or local rules.

Mansioni

• Maintain client and employee files and related paperwork.
• Ensure HIPAA confidentiality of patient information and agency records.
• Provide direct training as needed.
• Assign homecare aides based on Plan of Care.
• Monitor performance of homecare aides to ensure quality service.
• Communicate schedules and changes with clients and staff.
• Conduct in-home visits with a client monthly.
• Prepare and submit routine departmental reports.
• Record and maintain documentation of client condition and service.
• Record client satisfaction surveys and staff trainings where applicable.
• Handle client complaints and document issues for resolution.
• Educate and coach on changes to a client's Plan of Care.
• Maintain positive relationships with clients and staff.
• Ensure compliance with laws and company policies.
• Perform other duties as assigned.
