Job summary

CSI, a Help at Home company, is seeking a bilingual Care Coordinator in Summerfield, FL to support clients and caregivers, manage scheduling, and ensure exceptional client experiences within home health services.

The role emphasizes care coordination, documentation, HIPAA compliance, and collaboration with leadership. Regular travel to client homes is required; a compassionate, organized individual with English/Spanish fluency will thrive in this team.

Qualifications

• High school graduate or equivalent preferred; may require higher level of education or certification.
• Fluent in both English and Spanish.
• Basic computer literacy and typing skills.
• Experience in health care, preferably in home health; supervisory experience.
• Medicaid, Waiver, or Home Healthcare experience preferred.
• Other Requirements pursuant to state or local rules as applicable.

Responsibilities

• May maintain elements of the assigned clients' files, employee files and all related paperwork.
• Confidentiality of patient/client and agency information following HIPAA guidelines.
• May provide direct training.
• Assigns homecare aides based on the client's needs and Plan of Care.
• Monitors performance of homecare aides to ensure quality service.
• Communicates schedules and changes with clients, aides, referral sources, team leads, managers and branch manager.
• May conduct in-home visits with a client per month to maintain perspective and connections.
• Prepares and submits routine departmental reports.
• Records and maintains documentation of client's condition and service.
• May conduct and keep client satisfaction surveys and employee evaluations.
• Handles client complaints; documents issues for resolution; mandatory reporter for incidents.
• Provides education and coaching on changes to a client's Plan of Care.
• Maintains positive relationships with clients, aides, referral sources, team leads, managers and branch managers.
• Ensures compliance with laws and company policies.
• Performs other duties as assigned.
