You will collaborate with healthcare teams, support families, and ensure timely delivery of services while upholding privacy regulations.

Qualifications

• High school diploma or equivalent.
• 1+ years of customer service experience required.
• 1+ years of home healthcare experience preferred.
• Bilingual in English & Russian required.

Responsibilities

• Receive referrals from hospitals, physicians, and other providers and review documentation.
• Manage care coordination for a caseload and schedule caregiver shifts based on client needs.
• Communicate with patients, families, and caregivers and maintain up‑to‑date records.
• Collaborate with care teams, participate in meetings, and support service verification and billing.
