Job summary

MultiCultural Home Care - MA, a Modivcare Personal Care Service, is seeking a Bilingual Care Coordinator Associate fluent in Haitian Creole and English to ensure patient needs are met efficiently. You will handle referrals, coordinate caregiver schedules, and serve as the main contact for patients and families.

The role includes on-call rotation, working Monday–Friday 8:00 AM–5:00 PM, with a Brighton, MA office base.

Qualifications

• High school diploma or equivalent.
• 1+ years of customer service experience required.
• 1+ years of home healthcare experience preferred.
• Experience using HHAeXchange preferred.
• Bilingual in English & Haitian Creole required.

Responsibilities

• Referral and intake management: receive referrals, review documentation, determine eligibility, initiate intake.
• Care coordination and scheduling: manage caseload, create/schedule shifts, maximize coverage, coordinate with patients/families, track changes.
• Communication and information management: be primary contact, share relevant patient info, maintain records, document complaints/incidents, ensure privacy.
