Job summary

Care Finders Total Care, part of Modivcare Personal Care Service, seeks a Care Coordinator Supervisor to supervise coordinators, oversee intake and care coordination, and ensure timely service delivery. The role supports patients, families, and healthcare teams with care plans, updates, and documentation.

The candidate will mentor staff, participate in care conferences, and assist with billing, compliance, and quality improvement initiatives. Fluency in English and Spanish is required.

Qualifications

• High school diploma or equivalent.
• 3+ years of care coordination experience required.
• 1+ years of experience using a Home Care Platform or scheduling system such as HHA Exchange.
• Bilingual in English & Spanish required.

Responsibilities

• Supervise and support Care Coordinators, including assigning tasks, monitoring workloads, and ensuring compliance with policies and procedures.
• Oversee patient intake and care coordination processes, ensuring continuity, quality of care, and timely service delivery.
• Receive and review referrals from healthcare providers, ensuring documentation is complete and accurate.
• Manage caregiver scheduling and staffing coverage based on authorized hours, caregiver skills, and client preferences.
• Serve as the primary point of contact for patients, families, caregivers, and healthcare teams regarding care coordination needs.
• Communicate patient updates, care plans, and scheduling changes while maintaining accurate and confidential records.
• Participate in care conferences, resolve service barriers, and support billing and service verification processes.
• Mentor and provide guidance to care coordination staff on complex cases and day-to-day operations.
• Educate patients and families on available services, community resources, and care expectations.
• Support quality improvement initiatives, data collection, compliance efforts, and participate in on-call rotation as needed.
• Perform other duties as assigned.
