Job summary

Help At Home, LLC is seeking a bilingual Care Coordinator (English/Spanish) based in Sarasota, FL. The role focuses on caregiver scheduling, care coordination, and delivering excellent client experiences in a hybrid work environment with training conducted onsite.

The position requires strong organization, leadership, and communication skills, plus experience in home health care and HIPAA compliance. Training and mobility within the Sarasota area are expected.

Responsibilities

• Maintains client and employee files and related paperwork.
• Maintains confidentiality of patient/client information per HIPAA.
• May provide direct training.
• Assigns homecare aides based on client needs and Plan of Care.
• Monitors performance of homecare aides ensuring quality personal care.
• Proactively communicates schedules and changes with clients, aides, and managers.
• May conduct in-home visits to maintain client and caregiver connections.
• Prepares and submits routine departmental reports.
• Records and maintains documentation of client condition and service.
• Keeps records of client satisfaction surveys and employee trainings.
• Handles client complaints and reports significant issues for resolution.

Education

High school graduate or equivalent
