Job summary

Astyra Corporation is seeking a dedicated Bilingual Care Coordinator (English/Spanish) to help members access healthcare services, providers, and community resources. This non-clinical role focuses on coordinating care, supporting transitions, and removing barriers so individuals receive needed services.

You will work with members, providers, and community organizations to arrange appointments, coordinate transportation, and ensure continuity of care. Fluency in English and Spanish is required.

Qualifications

• Associate's degree in a healthcare-related field.
• Bilingual proficiency in English and Spanish required.
• 1–3 years in care coordination, social services, case management, or related field.
• Knowledge of healthcare systems and community resources.
• Strong communication and problem-solving skills.
• Ability to manage multiple priorities and maintain documentation.
• Proficiency in Microsoft Office and electronic case management systems.
• Ability to work independently with teams.

Responsibilities

• Assist members with navigating healthcare services, referrals, and community resources.
• Conduct non-clinical assessments to understand member needs and barriers.
• Coordinate initial provider appointments, transportation, and support services.
• Support members through transitions of care to maintain continuity.
• Document interactions, referrals, and follow-up in electronic systems.
• Communicate effectively with English- and Spanish-speaking members.
