Job summary

Aahealth is seeking a dedicated Bilingual Coordinator of Special Programs II for the REACH program in Annapolis, MD. The role involves coordinating healthcare services for adults and children, determining eligibility for Spanish-speaking applicants, and enrolling eligible individuals in REACH.

You will provide outreach and care coordination to REACH members, helping them access resources, improve health status, and navigate the system.

Qualifications

• Fluency in English and Spanish (written and spoken).
• Case management experience preferred.
• Data entry proficiency and basic computer skills.

Responsibilities

• Coordinate comprehensive healthcare services for REACH program participants.
• Determine eligibility for Spanish-speaking applicants and enroll them in REACH.
• Provide outreach and care coordination to non-compliant REACH members and help access resources.
• Educate REACH members on health status, self-care, and access to care.
• Engage with community providers and attend staff meetings and trainings.
