Job summary

Good Shepherd Services is seeking a dedicated and compassionate bilingual (Spanish-speaking) Care Coordinator to support youth in our Health Home program. The role guides participants and guardians through the health care system, addresses access barriers, and monitors interventions to ensure high-quality care coordination.

The candidate will report to the CARE MANAGER SUPERVISOR, HEALTH SERVICES, and work on-site at 685 E. 187th Street, Bronx, NY 10458.

Qualifications

• Bachelor's degree required with two years' experience.
• Bilingual Spanish/English candidates encouraged.
• Experience as a Case Planner, Care Coordinator, Case Manager, or Case Worker.
• Experience in child welfare and related systems.

Responsibilities

• Secure care management enrollments and conduct needs assessments.
• Develop and monitor the IPC to coordinate services.
• Coordinate services across medical, behavioral health, and social domains.
• Maintain documentation in Care Management Portal and EHRs.
• Assist clients in obtaining public benefits.
