La Casa de Salud (LCDS) in the South Bronx seeks a Care Coordinator to act as the liaison between providers and patients, schedule appointments, and ensure timely follow-ups for results and referrals.
You will enter data in the EMR, conduct outreach via phone and mail, maintain compliant paperwork, and facilitate connections to health services for vulnerable communities.
Bilingual English/Spanish preferred; HS Diploma required; two years of care coordination experience.

The Care Coordinator, under the supervision of the Program Director, is the liaison between both the provider and the patient. Schedules appointments for both internal and external services. Conduct follows ups to ensure patient kept appointment and results and/or consultation reports are available for the patient's follow-up visit with the provider.
