Job summary

CSI, a Help at Home Company, is seeking a Care Coordinator based in Tampa. The role focuses on caregiver scheduling, care coordination, and delivering exceptional client experiences in home health services.

The position requires bilingual English/Spanish abilities, strong organizational skills, and a passion for improving client outcomes. Office hours are standard, with daily travel to client sites as needed.

Qualifications

• High school diploma or equivalent preferred.
• Fluent in English and Spanish.
• Basic computer literacy and typing skills.
• Experience in health care, preferably in home health.
• Staffing/scheduling experience preferred.
• Medicaid, Waiver, or Home Healthcare experience preferred.

Responsibilities

• Maintain client and staff files and related paperwork.
• Respect HIPAA guidelines and maintain confidentiality of records.
• Provide direct training where applicable.
• Assign homecare aides based on client needs and Plan of Care.
• Monitor performance of homecare aides for quality service.
• Communicate schedules and changes with clients, aides, and managers.
• May conduct monthly in-home visits to engage with clients and caregivers.
• Prepare routine departmental reports and maintain accurate documentation of client status.
