Qualifications

• High School Diploma required.
• Minimum 2 years of experience in medical management.
• Capacity to interpret health plan benefit decisions.
• Bilingual English/Spanish required.
• Certification as a Medical Assistant preferred.

Responsibilities

• Collect relevant clinical data to support the care planning process.
• Care Plan support and transition interventions to optimal health levels.
• Collaborate with providers across settings to ensure outcomes.
• Provide transition of care interventions as required.
• Facilitate care coordination, discharge planning, and health education.
• Link to community resources to address social determinants of health.
• Adjudicate referrals according to defined business rules.
• Adheres to Quality Management Committee policies.
