Job summary
The Wonderful Center for Health Innovation is seeking a Behavioral Health Therapist based in Lost Hills, CA, who will travel to wellness centers across the Central Valley. You will join our onsite primary care team, collaborating with the PCP, psychiatric consultant, and other mental health providers to improve patients' physical and mental health.
You will screen for mental health and substance use, provide education, coordinate care, and support referrals and treatment plans.
Qualifications
- Licensed CA LCSW or LMFT is required.
- Fluent in Spanish to communicate with patients and families.
- Demonstrated ability to collaborate effectively in a clinic team.
Responsibilities
- Coordinate mental health care with the patient’s primary care provider and other treating providers.
- Screen and assess patients for mental health, substance use, and social needs.
- Provide patient education about mental health and treatment options.
- Monitor patient symptoms and treatment side effects.
- Facilitate referrals for psychosocial treatments and community services.
- Support psychotropic medication management and treatment adherence.
- Participate in weekly caseload consultations with the psychiatric consultant.
