Behavioral Health Case Manager

We are seeking a compassionate and mission‑driven Behavioral Health Case Manager to provide direct services and coordinate care for patients with complex behavioral health needs. Under the supervision of the Program Manager, this role focuses on patient‑centered, evidence‑based care coordination, outreach, and rehabilitative services across clinic and community settings.

What You’ll Do:

• Provide direct case management, rehabilitative services, outreach, and care coordination
• Conduct comprehensive psychosocial assessments and develop individualized treatment plans
• Collaborate with Behavioral Health, Primary Care, and multidisciplinary teams
• Facilitate referrals, warm handoffs, follow‑ups, and access to in‑person and telehealth services
• Maintain accurate documentation in the electronic health record

What We’re Looking For:

• Bachelor’s degree in Psychology, Social Work, or related field (or equivalent experience)
• 1+ year of behavioral health case management experience
• Bilingual/Bicultural Spanish‑English or Arabic‑English preferred
• Strong communication, documentation, and collaboration skills
• Valid CA Driver’s License and ability to work in community settings

Base pay range

$31.00/hr - $39.00/hr
