Job summary

Personal Touch Home Care is seeking an After Hours Coordinator to join our Brooklyn office. This in-person, full-time role coordinates and schedules patient care, handling inbound/outbound calls from patients and families. Must be bilingual and available weekends with a regular 2:00 PM–10:00 PM shift, plus holiday coverage.

You will ensure timely service delivery, monitor caregiver availability, and document interactions and scheduling changes within required systems.

Qualifications

• Flexible schedule with weekend availability.
• 2:00 PM–10:00 PM weekday shift with potential earlier start.
• Bilingual English/Spanish preferred and strong written/verbal communication.

Responsibilities

• Answer calls and provide accurate, satisfactory responses to inquiries.
• Notify patients/families of service changes and coordinate care.
• Schedule and coordinate patient care staff per policy.
• Maintain daily updates on employee absences, cancellations and illnesses.
• Assist in monthly scheduling and time summaries and pre-billing workflows.
• Document patient and caregiver records accurately and timely.
• Communicate with contracts, payors, and other entities as needed.
