Job summary

Personal Touch Home Care is seeking a Scheduling Coordinator to coordinate and schedule patient care in Brooklyn, NY. This full-time, in-person role requires bilingual English and Mandarin or Spanish proficiency and occasional weekend work.

Pay ranges from $21.00 to $25.00 per hour, with opportunities for growth and a supportive benefits package. The coordinator will match clients with caregivers, communicate with families and clinical staff, and ensure regulatory compliance across the care

Qualifications

• Proficient in Microsoft 365.
• HHAX experience preferred.
• 1 to 3 years' experience as a Home Care Coordinator.
• Ability to multitask.

Responsibilities

• Coordinate the scheduling and ongoing management of home care services.
• Match clients with appropriate caregivers based on home care needs, availability, and skill set.
• Communicate regularly with clients, families, caregivers, and clinical staff.
• Monitor service delivery, address issues, and ensure continuity of care.
• Maintain accurate documentation and ensure compliance with state and agency regulations.
• Respond to client concerns and resolve service-related issues promptly.
• Participate in quality assurance and performance improvement activities.
• Coordinate with patients, families and insurance contacts to ensure services are scheduled and plans and any changes are communicated promptly and effectively.
• Manage scheduling of a patient caseload and associated responsibilities, such as pre-billing, attendance, and visit verification.
• Accurate and timely documentation in patients' and caregivers' records as applicable.
• Ability to work occasionally on weekends and holidays as needed.
• Identify and escalation issues to supervisors.
• Perform other duties as assigned.
