Job summary

Gaudenzia, Inc. in Pennsylvania seeks an Addictions Counselor (licensed or certified) to support individuals on the recovery journey.

You will manage a caseload, conduct evaluations, develop treatment plans, and document care in the electronic health record, with emphasis on serving mono-lingual Spanish-speaking clients and occasional evening shifts. Qualifications

• Bachelor's degree with one year of clinical experience.
• Bachelor's degree with two years of clinical experience.
• Bachelor's degree with three years of clinical experience; or Master’s degree with related field and minimal experience.

Responsibilities

• Be responsible for a caseload of clients in accordance with program requirements and state regulations.
• Complete required evaluations and assessments, including screening, intake, clinical assessments, ASAM and biopsychosocial histories.
• Develop individual treatment plans upon client admission.
• Document all clinical services and interventions in the client’s electronic health record and complete monthly treatment reports.
• Maintain accurate electronic health records and ensure timely task completion within 24 to 72 hours of service delivery.
• Conduct individual and group sessions based on treatment plan goals and level of care.
• Participate in funding source reviews and make treatment/length-of-stay recommendations.
• Monitor client progress and adjust treatment plans as needed.
• Coordinate with agency and community resources to ensure a thorough treatment plan.
• Assist clients with developing discharge/continuing care plans and connecting to resources for goals during and after treatment.
• Follow up with clients after discharge per regulatory guidelines until established with other case management/social services.

Education

Bachelor's degree with one (1) year of clinical experience

Bachelor's degree with two (2) years of clinical experience

Bachelor's degree with three (3) years of clinical experience; Master's degree in Social Work, Psychology, Chemical Dependency, Counseling, or Nursing with no experience required; or Master's degree in related field with one (1) year of clinical experience
