Job summary

Gaudenzia, Inc. in Erie, PA, seeks a full-time Addiction Counselor for our outpatient program. You will manage a caseload, conduct assessments, develop treatment plans, and deliver individual and group sessions to support recovery from substance use disorders.

Qualifications

• State-specific counselor licensure/education requirements.
• Degrees include Social Work, Psychology, Addiction, Counseling, Human Services, or related fields.
• Experience levels vary by Counselor level (I–III).

Responsibilities

• Manage a caseload of clients per program requirements and state rules.
• Perform evaluations and assessments (screenings, intake, biopsychosocial histories).
• Develop and complete individual treatment plans at admission.
• Document all clinical services in the electronic health record.
• Maintain timely task completion and monthly reports.
• Lead individual and group sessions according to treatment plans.
• Participate in funding reviews and adjust treatment lengths as needed.
• Monitor progress and update plans accordingly.
• Coordinate with resources to ensure a through treatment plan.
• Assist with discharge planning and link to continuing care resources.
• Follow up after discharge per regulatory guidelines.
