Job summary

Gaudenzia, Inc. is seeking counselors in Pennsylvania to support clients with substance use disorders and co-occurring mental health needs.

You will manage a caseload, perform evaluations and assessments, develop individual treatment plans, and document services in the electronic health record, with timely updates. Qualifications

Be responsible for a caseload of clients in accordance with program requirements and state regulations.
Complete required evaluations and assessments, including screening, intake, clinical assessments, ASAM and biopsychosocial histories.
Compete individual treatment plans upon client admission.
Document all clinical services in the electronic health record and complete monthly treatment reports.

Responsibilities

Maintain accurate electronic health records within 24–72 hours of service delivery.
Conduct individual and group sessions based on treatment plan goals and level of care.
Participate in funding source reviews and recommend treatment and lengths of stay.
Monitor client progress and adjust treatment plans as needed.
Coordinate with agency and community resources to ensure through treatment plan.
Assist clients with discharge/continuing care planning and connecting to resources.
