Job summary

Qualifications

  • Bachelor’s degree from an accredited school in social work, psychology, counseling, pastoral education, or related field.
  • Six months experience in bereavement programs or support activities.
  • Current state driver’s license and proof of valid automobile insurance

Responsibilities

  • Develops a bereavement plan of care that notes the kind of bereavement services to be offered and the frequency of service delivery.
  • Provides bereavement services including spiritual support and any other counseling services for the client and family consistent with the bereavement plan of care.
  • Coordinates the assessment and delivery of bereavement and grief counseling needs and services (via phone, office visits, home visits, groups and ongoing follow-up) to ensure timely and appropriate services are provided to client/family/caregivers.
