Position Summary: Ensures the overall administration and implementation of bereavement services provided while adhering to Medicare and state-specific regulations. Provides assistance with issues related to grief and loss, as well as spiritual support and any other counseling services for the client and family in keeping with their desires and belief system.
1.Develops a bereavement plan of care that notes the kind of bereavement services to be offered and the frequency of service delivery.
2.Provides bereavement services including spiritual support and any other counseling services for the client and family consistent with the bereavement plan of care.
3.Coordinates the assessment and delivery of bereavement and grief counseling needs and services (via phone, office visits, home visits, groups and ongoing follow-up) to ensure timely and appropriate services are provided to client/family/caregivers.
