Job Opportunity: Registered Nurse Care Coordinator

The Opportunity

We are seeking experienced Registered Nurses to join our Clinical Programs team as Care Coordinators supporting the RITH program that Amplar Health delivers to patients in their homes.

This is a phone-based role where you will be trained in all aspects of the RITH program to support participating patients and community nurses.

Using your clinical skills, you will ensure a comprehensive, patient-centered management plan by:

  • Conducting initial phone assessments, care planning, and goal setting.
  • Coordinating in-home services to support patient rehabilitation and recovery.
  • Monitoring patient progress and updating care plans as needed.
  • Building and maintaining effective stakeholder relationships, including primary and secondary care organizations and support teams.
  • Liaising with healthcare professionals and specialists.
  • Providing telephone support, education, and advice.

This is a full-time, permanent position primarily working Monday to Friday (8:30 am - 5:00 pm WA time), with occasional weekend shifts. It is a fully remote role, suitable for candidates based in WA with a strong understanding of the WA healthcare system and services.

About You

You are an outstanding communicator, comfortable liaising with people via phone (inbound and outbound), with strong people skills essential for success. Ideally, you have experience supporting post-op patients and managing post-surgical complications. You are empathetic, observant, attentive, trustworthy, and committed to patient safety and comfort. Client and goal-focused, you are driven to achieve positive outcomes. Additionally, you will have:

  • At least 2 years’ experience as a Registered Nurse with unrestricted AHPRA registration, preferably in an acute setting.
  • Experience in care coordination or phone-based roles is advantageous.
  • Strong customer service and relationship management skills.
  • Excellent clinical reasoning abilities.
  • Capacity to work in a fast-paced environment.
  • Proficiency with computer systems for accurate data entry into electronic medical records.
  • A reliable internet connection.
