Eleve Wellness seeks a Behavioral Therapist in New Jersey to deliver clinical assessments and tailored treatment plans for individuals facing mental health and substance use challenges. The role emphasizes evidence-based therapies, including CBT, DBT skills, MI, and relapse prevention, within PHP and IOP settings.

• Master's degree in Social Work, Counseling, Psychology or closely related field.
• Current New Jersey clinical or counseling credential (LAC, LPC, LSW, LCSW or equivalent).
• Experience in behavioral health or substance use treatment preferred.
• Experience in PHP/IOP/outpatient settings preferred.
• Experience facilitating therapeutic groups preferred.

Responsabilidades

• Conduct initial behavioral health assessments and ongoing clinical evaluations.
• Develop, implement, and update individualized treatment plans.
• Provide individual counseling and facilitate groups for PHP/IOP clients.
• Maintain accurate and timely electronic health records (EHR) and progress notes.
• Coordinate discharge planning and continuity-of-care with the treatment team.
• Follow HIPAA, licensing and payer requirements; maintain professional boundaries.
