Job summary

Qualifications

• High School Diploma is required.
• Valid Driver's license is required.
• Experience working with people who have mental health diagnoses, intellectual/developmental disabilities, or substance use disorders.

Responsibilities

• Assess individuals for hopes, dreams and desires and provide supports to help achieve them.
• Assist in participating fully in the community consistent with the person’s interests.
• Protect and promote the rights of people receiving services and support advocacy efforts.
• Provide emotional, physical, and social support as needed.
• Build person-centered, positive relationships using empowering approaches.
• Document progress and complete required forms and goals.
