Job summary

Birch Family Services is seeking an Assistant Clinician to collaborate with residential and day habilitation teams under the guidance of the BIS, RN and Manager. You will help identify needs, develop goals, and monitor progress within CFA/Life Plan frameworks.

You will assist with assessments, data collection, and staff training to ensure consistent, evidence-based programming across services. Regular IDT participation is expected to support person-centered planning.

Qualifications

• Bachelor’s degree in Psychology, Social Work, Education or related Human Services field.
• Minimum of 3 years’ experience in OPWDD related field.
• Should have the ability to work a flexible schedule if needed to accommodate working with the individuals in the residence and to attend team meetings.

Responsibilities

• Work directly with residential or day habilitation staff under supervision of the BIS, Manager and Coordinator.
• Identify behavioral, psychiatric, social, vocational, health, nutritional and mobility needs and bring to team attention.
• Assist BIS, RN or Manager/Coordinator to conduct assessments and observations.
• Develop goals/outcomes in CFA or Life Plan and monitor progress.
• Design data collection systems to monitor progress and measure outcomes.
• Ensure programming is implemented in accordance with agency policy.
• Monitor habilitation paperwork and shares reports with providers.
• Coordinate with IDT to obtain proper approvals for interventions.
