Job summary

Birch Family Services is seeking an Assistant Clinician to work on-site in Brooklyn, NY, supporting individuals in residential or day habilitation settings. You will collaborate with the BIS, RN and IDT to assess needs and contribute to the CFA/life plan, ensuring interventions are properly documented and implemented.

The role requires a Bachelor’s degree in a related field, at least 3 years in an OPWDD-related setting, and flexibility to attend team meetings and adapt to a changing schedule.

Qualifications

• Minimum of 3 years’ experience working in OPWDD related field and preferably experience in a residential, health or treatment related setting.
• Should have the ability to work a flexible schedule if needed to accommodate working with the individuals in the residence and to attend team meetings.

Responsibilities

• Work with BIS and IDT to assess behavioral, psychiatric, social, health and mobility needs of individuals.
• Develop goals/outcomes in CFA/life plan and review data for accuracy and progress.
• Design data collection systems with the interdisciplinary team to monitor progress.
• Assist with staff training to ensure proper implementation of clinical interventions.
• Facilitate CFA/life plan meetings and coordinate staff action plans.
