Job summary

The Wonderful Company is seeking a Behavioral Health Therapist based in Lost Hills, CA, who will travel to other wellness centers in the Central Valley and join our onsite primary care team.

You will coordinate mental health care with the patient’s PCP and psychiatric consultant, screen for disorders, educate patients on treatment options, monitor symptoms, and facilitate community referrals. The role offers 90,000–100,000 USD yearly, with a discretionary bonus and a monthly car allowance.

Qualifications

• Current/valid LCSW or LMFT license issued by the CA Board of Behavioral Sciences.
• Must speak Spanish fluently.
• Preferred experience working with adults, teens, and pediatric patients.
• Knowledge of social service agencies, state regulations, and APA standards.
• Demonstrated ability to collaborate effectively in a team setting.

Responsibilities

• Support and coordinate mental health care with the patient's PCP and other providers.
• Screen and assess patients for mental health, substance use disorders, and unmet social needs.
• Provide patient education about mental health and treatment options.
• Monitor patients for changes in clinical symptoms and treatment side effects or complications.
• Develop relationships with community providers and referrals for services outside the primary care clinic.
• Support psychotropic medication management prescribed by PCPs.
• Provide brief interventions (e.g., Behavioral Activation, Motivational Interviewing).
• Provide or facilitate referrals to evidence-based psychosocial treatments.
• Participate in weekly caseload consultations with the psychiatric consultant.
• Facilitate treatment plan changes with PCP and psychiatric consultant.
• Complete relapse prevention plan with patients in remission.
• Travel required to Lost Hills clinic, Delano clinic, Student Wellness Centers, and Mobile clinic.
• Perform other job-related duties as assigned.
