Community Health Network is seeking a Behavioral Health Therapist at the associate licensed level to work with individuals and families from diverse backgrounds. This role emphasizes evidence-based practices and culturally competent care.

You will conduct assessments, provide individual/group/family therapies, and collaborate on treatment planning and discharge processes, ensuring ethical, outcome-focused care.

Formación

• License eligible for the LCSW, LMHC, or LMFT in Indiana.
• Master's degree in a mental health related field.
• 2+ years of experience providing mental health treatment.

Responsabilidades

• Provide psycho-social assessments, individual, family and group therapy, and crisis intervention.
• Collaborate with clients to develop personalized treatment plans using evidence-based practices.
• Provide therapy sessions (individual/family/group) to reduce distress and improve functioning.
• Develop and implement discharge plans aligned with client needs.
• Collaborate with interdisciplinary teams to coordinate care and referrals.
• Maintain confidential documentation of client progress and outcomes per standards.
