Qualifications

• Bachelor's degree in social work or equivalent.

Responsibilities

• Provides evaluation, counseling and therapeutic services to patients and families.
• Makes treatment recommendations and acts as liaison with treatment network.
• Maintains communication with referral sources and families about progress and discharge plans.
• Supports patient and family throughout treatment and coordinates care with the interdisciplinary team.
• Develops individual treatment plans based on gathered information.
• Coordinates Aftercare planning and post-treatment referrals.
• Maintains compliance with behavioral health guidelines and regulatory standards.
