Responsibilities

• Provides evaluation, counseling and therapeutic services to chemically dependent patients/families.
• Makes recommendations for treatment and acts as liaison with the treatment network.
• Maintains communication with referral sources, families and others involved in treatment.
• Coordinates patient care with the interdisciplinary team and develops individual treatment plans.
• Facilitates programming for court-ordered referrals and supports Aftercare planning.
