Carle Health is seeking a behavioral health professional to provide in-home and community-based services in Illinois. You will assess, plan, and implement treatment, respond to crises, and coordinate with physicians, families, and community resources to support clients with severe mental health concerns.

The role emphasizes strong documentation, effective communication with medical staff, and advocacy for clients within a multidisciplinary team.

Mansioni

Provides community-based/in-home behavioral health services for individuals with behavioral health concerns.
Provides transportation to clients when clinically appropriate.
Completes assessment and treatment plans with clients and requests consultations as needed.
Provides crisis assessment and intervention and follows up post-crisis.
Offers individual and group services including monitoring daily living activities and advocacy.
Maintains records per organizational and regulatory standards including Joint Commission and Medicaid.
Coordinates treatment goals with community agencies, primary care, and managed-care entities.
