Job summary

Carle Health in Peoria, IL seeks a Counselor to deliver evaluation, counseling and therapeutic services to inpatients and outpatients with substance use and behavioral health needs. You will work within an interdisciplinary team to develop treatment plans and coordinate care with families and referral sources.

The role requires multiple state licenses and certifications within defined timelines (CRADC, CAADC, LPC, CADC, CSADC, LSW, LCSW/LCPC).

Qualifications

Certifications must be current in IAODAPCA/IDFPR as listed.
Licensure timelines are within 1–2 years depending on certification.
Education includes master's degrees in psychology, counseling or social work.

Responsibilities

Provides evaluation, counseling and therapeutic services to chemically dependent patients/families assessing social, psychological, emotional, financial, cultural problems and chemical abuse history.
Makes recommendations for treatment and acts as a liaison and patient/family advocate with the treatment network.
Maintains open communication with referral sources, families, or others involved in the patient’s treatment regarding progress, discharge plans and aftercare plans.
Coordinates the patient’s care with the interdisciplinary treatment team.
Develops individual treatment plans based upon information gathered.
Facilitates programming for court ordered referrals.
Assists patient and family with adjustment to illness, motivation for treatment and planning for post treatment care; completes Aftercare Plan with patient.
Documents per department guidelines and stays current with managed care and precertification processes.
