Responsibilities

• Write, review, evaluate, and update individualized program and support plans based on individual needs and interdisciplinary recommendations.
• Develop measurable outcomes and monitor progress using tracking data and progress notes.
• Complete IPP/ISP documentation, progress notes, reports, and other required clinical records.
• Provide individual therapeutic services based on identified needs and treatment plans.
• Collaborate with QIDPs, program leadership, direct support professionals, and medical providers.
